Thats kinda what the Not Sure category is for. But if you are really sure about it you can put it in Yes as well.

No, because I would guess that people who are ND are more likely to click on it which would bias the results.

A lot of neurodivergency is self diagnosed even if you got the official diagnosis, because psychologist have to trust you that you have enough self awareness and honesty to describe certain symptoms correctly, and vice versa you have to trust that psychologist has enough knowledge in the field, experience and awareness to gauge whether those symptoms are correctly described and whether you are being self aware enough; if you are only mildly neurodivergent then you are pretty much always situated on the boundary of self diagnosis

Adhd is comfort in moving your eyes really fast. Thats why adhd individuals play competitive video games. Its survival mechanism from being a baby.

I was diagnosed with ADHD as a kid and took medication till the age of 18. I don't know if I really do have ADHD or was just a kid being a kid when I was diagnosed that day in the office. I worry about having taken medication for ao long and the long term harm this could've done. 

Unless your kid shows CLEAR signs of some condition I don't believe in medicating young minds. Nurture, love and be there for them, let them grow up to be the people they where meant to be free of the mind fog  of medication.

But kids often suffer in school a lot when they have adhd. Its quite the predicament for parents.
