Thats kinda what the Not Sure category is for. But if you are really sure about it you can put it in Yes as well.

No, because I would guess that people who are ND are more likely to click on it which would bias the results.

A lot of neurodivergency is self diagnosed even if you got the official diagnosis, because psychologist have to trust you that you have enough self awareness and honesty to describe certain symptoms correctly, and vice versa you have to trust that psychologist has enough knowledge in the field, experience and awareness to gauge whether those symptoms are correctly described and whether you are being self aware enough; if you are only mildly neurodivergent then you are pretty much always situated on the boundary of self diagnosis
