Well, we inherited this system from USSR, in which all healthcare was completely free. But now we also had an emergence of capitalism and people creating a bunch of private clinics. Doctors get paid like 2-5x there, compared to government clinics, so the best doctors go here leaving the vast amount of population with stable mediocrity (unless they have $ for paid clinics)
