sustained motivation

when I make a decision that i'm going to go to the gym, say, 4 times a week for the next three months, although it seems like I could do it using will-power, I never really make it past one or two weeks. has anyone got any advice on how to maintain sustained motivation throughout a three month period?

Have a no excuses mindset, Take 100% responsibility and have someone hold you accountable and make it a habit.

Don't rely on motivation. Motivation comes and goes. It never lasts.

Just take one right action such as getting yourself out the front door and walking to the gym. The motivation kicks in once you get to the gym. Or just get to the gym and say I'm going to just do the warm up. You'll then feel motivated to do the whole thing since you're already there.

Discipline, leads to motivation, leads to flow state, leads to results

once you make it past taking your gym bag and getting outside of house, the rest is easy. Convincing your lazy body to go is the hardest part.
